Mindfulness Inventory


Which statements are an accurate reflection of your inner experience? Record the number of the statement and whether or not you feel the statement describes your own opinion of what is generally true for you with a “yes” or a “no”.

  • 1. I am often on autopilot without much awareness of what I’m doing or what is going on around me.
  • 2. I often find myself thinking about the future or the past.
  • 3. I tend to walk quickly to get where I’m going without noticing what is happening around me.
  • 4. I miss details or make mistakes because I rush through activities without being really attentive.
  • 5. When I am busy or stressed I often hold physical tension in my body resulting in tight shoulders or an upset stomach for example.
  • 6. In stressful situations, I find myself immediately reacting without giving things much thought.
  • 7. I frequently find myself doing many tasks at once.
  • 8. I check my phone often during meetings or conversations with friends and loved ones.
  • 9. I find myself thinking about what I want to say next in a conversation rather than listening.
  • 10. It is common for me to forget someone’s name seconds after meeting them.
  • 11. I find myself eating when I am stressed or upset even though I am not hungry.
  • 12. I snack or overeat without being aware because I am often preoccupied.
